<Abstract>
Background
Submental liposuction is a widely performed cosmetic surgery procedure aimed at improving the contour of the jawline and neck. However, methods for quantitatively predicting the amount of fat to be aspirated preoperatively are not well-established, and preoperative explanations and surgical planning have relied on the surgeon's experience. This study aimed to predict the aspiration volume in submental liposuction preoperatively using simple physical measurements obtainable during outpatient consultations.
Methods
A single-center prospective observational study was conducted on 63 cases who underwent submental liposuction using the syringe technique by the same surgeon between January and August 2025. Preoperative evaluations included vertical and horizontal distances of the submental area, subcutaneous fat thickness measured by skinfold calipers, palpated subcutaneous fat hardness, and BMI. The correlation between these indicators and the actual aspiration volume was analyzed, and a multiple regression model was constructed to predict aspiration volume.
Results
The main factors correlated with aspiration volume were subcutaneous fat thickness, vertical distance, BMI, and fat hardness. The final model demonstrated that aspiration volume could be predicted with a certain degree of accuracy using a regression equation based on subcutaneous fat thickness, vertical distance, BMI, and hardness. The prediction formula developed in this study is as follows: Predicted Aspiration Volume (mL) = -7.3587 + 0.7904 × Subcutaneous Fat Thickness (mm) + 0.3084 × Vertical Distance (cm) + 0.8934 × BMI - 4.3066 × Hardness Score (1–3)
Conclusion
This study developed a regression equation to predict preoperative aspiration volume in submental liposuction from simple indicators obtainable during outpatient consultations. This may be helpful for preoperative counseling, surgical planning, training junior surgeons, and standardizing procedures.
