Hitachi and Kyoto University Develop a Community-Based Inclusive Society Model Through 'Social Prescribing' to Connect Residents and Communities

Hitachi, Ltd., through joint research with Kyoto University, has formulated a community-based inclusive society model based on 'social prescribing' to connect residents and communities. This model systematizes the mechanism of social prescribing, which links residents with medical, welfare, and community resources by integrating high-risk and population approaches. By 'prescribing' social connections, it aims to extend healthy lifespans, resolve isolation, and contribute to optimizing healthcare costs and creating a sustainable, inclusive society. A key feature is its dual-cycle structure, where individuals, supported by 'link workers,' participate in community activities, and the outcomes are fed back to strengthen community-wide preventive measures.
