This travel product will be introduced via email to customers who make a reservation for a health checkup through this form.

For details, please check the email sent by "JA Nagano Health Association Hokushin General Hospital" after making a health checkup reservation.

○ Notes

・Accommodation under this special plan is only available at "Yuchitan Onsen Ichicha no Komichi Miyu no Yado."

・Travel applications can be made up to one month before the departure date.

・The results of the train booking for the outbound and return trips will be notified by email on the respective release dates. Please proceed with the payment procedure as soon as you receive the email. If not completed within the deadline, the booking will be automatically canceled, so please be careful. Note that in such cases, the cancellation of the health checkup must be handled by the customer themselves.

・The sale of travel products will be terminated as soon as the reservation slots for the health checkup reach their limit.

・This is a travel product exclusively for health checkup recipients, so dinner on the first day will be provided by 9:00 PM, and breakfast on the day of the checkup will not be available.

・Only one-way transportation is provided from "Yuchitan Onsen Ichicha no Komichi Miyu no Yado" to "JA Nagano Health Association Hokushin General Hospital" on the day of the health checkup. Please arrange the return trip yourself.

・If you wish to cancel the travel product, the prescribed cancellation fee will be charged. Please proceed with the cancellation procedure for both the health checkup and the travel product.

・Payment methods are limited to credit card payment in full. Cash payments at convenience stores, Eki Tabi Concierge, or bank transfers are not accepted.

・The examination fee for the health checkup, accommodation tax, and transportation costs during the stay are not included.

・Changes to the application content (such as partial cancellations or reductions, or changes to train reservations) will not be accepted. If you wish to make changes after applying, please cancel and make a new application. If the payment is completed (contract is established), the prescribed cancellation fee will apply. Also, there may be cases where reservations cannot be made at the same price. Please be careful when changing the date of the health checkup.
